This screen will allow the user to print the required 1094-C and 1095-C Forms.

Required Fields

Plan start month- Will default to 01- January. Allowed values are 00-12.

  • "Verification Report" - this report can be used to validate all of the information that will on the 1094-C and 1095-C reports. See the 1094-C and 1095-C Verification List for a complete list of errors. If work files have not been generated, a second report will also print showing all full time employees without coverage. See IRS Form 1094-C for more details on which employees are considered full time employees without coverage.
  • "Generate Work Files" - this will send data for your 1094/1095-C selections to the ACA Work Files for review. User will be required to enter initials indicating they have printed and reviewed the verification report prior to generating the work files.
  • "Refresh Work Files" - this will clear the existing work files and regenerate them. A warning message will appear for the user if the files have already been accepted. If the user selected yes, the submission will be cleared.
  • "1094-C and 1095-C" (runs both the 1094-C and 1095-C forms)
  • "Create 1094-C Correction" (if only 1094-C corrections are necessary, this will create it for the year and after attaching the receipt number, running the 1094-C and 1095-C selection will ONLY create 1094-C corrections)

Other Fields

Supported 1095-C paper stock - This determines what paper stock the form is printed on.

  • Landscape - Preprinted Harris Stock
  • Portrait - Blank Harris Stock

If 'Landscape' is chosen, report margin options will become available. Certifications of eligibility will become available if a paper stock is selected. This is required if "1094-C and 1095-C" is selected in the Select Report drop down.

Report margins in inches - Note: These options are ONLY available when landscape is the stock option

  • "Top Margin" - Sets the top margin of the 1094-C / 1095-C reports. Default setting is 0.4 inches.
  • "Left Margin" - Sets the left margin of the 1094-C / 1095-C reports. Default setting is 0.4 inches.

XML file location - The location and file name of the XML file that will be generated. If this is blank, a file will not be created. This option is only available if 1094-C or 1094-C and 1095-C are selected in the s​elected report dropdown.

Authoritative transmittal - The IRS requires a single 1094-C submission designated as the authoritative transmittal that contains all required employees.Check if creating your authoritative transmittal. Certifications of eligibility will become available if this is checked.

Total number of Forms 1095-C filed on behalf of your organization - This only becomes available if Authoritative transmittal is checked. If submitting forms 1095-C in batches or another party is filing some of your organization's 1095-C forms, enter the total number of forms 1095-C submitted by and/or on behalf of your organization for the reporting year.
