Introduction​

This screen will allow the user to print the required 1094-B and 1095-B Forms. It is possible to print a 1094 form without printing 1095s by using the Report select drop down box.

To submit the XML files created by this screen, follow the Submitting IRS 1094/1095 B and C XML Files Instructions.

Explanation

To complete a reporting year, the user must click print, generate XML files successfully and answer yes to the message box that pops up asking if the IRS submission has been accepted or accepted with errors.

Required Report Selections

Select reporting year - determines which reporting year the report will use to generate forms.

Origin of the policy - this corresponds to box 8 on the IRS Form 1095-B. This also determines which employees the report will create 1095-B forms for.

List of all possible options for Origin of the policy

  • A Small Business Health Options Program (SHOP) - Not supported
  • B Employer-sponsored coverage
  • C Government-sponsored program
  • D Individual market insurance
  • E Multiemployer Plan
  • F Other designated minimum essential coverage
Configuration Note:
If option 'B' is selected - Benefits need to be configured with the 'Employer-sponsored self-insured health plan' attribute 
If option 'E' is selected - Benefits need to be created with the 'Multiemployer Arrangement' attribute

Who gets a 1095-B?
The "Origin of the Policy" option determines which employees a 1095-B will get a 1095-B.

If options C, D, or F are selected,

all employees with a health benefit,

active during the reporting year (based on benefit start and end dates)

with 'Employer-sponsored self-insured health plan' and 'Multiemployer Arrangement' unchecked and 'Decline offer of coverage' unchecked

...Will get a 1095-B

If options B is selected,

all employees with a health benefit,

active during the reporting year (based on benefit start and end dates)

with 'Employer-sponsored self-insured health plan' checked and 'Decline offer of coverage' unchecked

...Will get a 1095-B

If options E is selected,

all employees with a health benefit,

active during the reporting year (based on benefit start and end dates)

with 'Multiemployer Arrangement' checked and 'Decline offer of coverage' unchecked

...Will get a 1095-B

  • "Verification Report" - this report can be used to validate all of the information that will be on the 1094-B and 1095-B reports. A complete list of errors is available here. All errors must be fixed before submission to the IRS AIR system.
  • "Generate Work Files" - this will send data for your 1094/1095-B selections to the ACA Work Files for review. User will be required to enter initials indicating they have printed and reviewed the verification report prior to generating the work files.
  • "Refresh Work Files" - this will clear the existing work files and regenerate them. A warning message will appear for the user if the files have already been accepted. If the user selected yes, the submission will be cleared.
  • "1094-B and 1095-B" (runs both the 1094-B and 1095-B forms)

Stock options - Disabled when "Verification report" is selected

  • "Landscape - Preprinted Harris Stock"
  • "Portrait - Blank Harris Stock"

XML File Location - Determines the output location of the XML file.

Report margins in inches - Note: These options are ONLY available when landscape is the stock option

  • "Top Margin" - Sets the top margin of the 1094-B / 1095-B reports. Default setting is 0.5 inches.
  • "Left Margin" - Sets the left margin of the 1094-B / 1095-B reports. Default setting is 0.5 inches.

XML Transmission and Manifest Files - these files are generated with the same data that is included on the 1094-B so the information can be submitted to the IRS electronically.

NOTE: The IRS only accepts 1094/95 XML files with 1095-B data included so if the 1094-B only option is selected, the XML file will NOT be created.

Troubleshooting

Dependents: the report requires the dependent to have either an SSN or Date of Birth. If both of those are blank then the dependent will not appear on the report.
